We all know an aged face when we see it, but most of us can not really say what makes us look old. It is clear that grey hair, age spots and wrinkles would make a person look older. But let’s look at someone like Joan Rivers. She has no wrinkles, no age spots and perfectly-coiffed blonde hair. Yet we all know that she clearly looks older than any average thirty year old woman we might see in the office or the mall. Youthful faces are full while older faces are thin and hollow.

In the past. Plastic Surgeons would simply cut away excess skin and suck out fatty deposits from the face to correct an aging face. Eyelid lifts were simply about removing as much skin and fat as possible. In extreme cases, the eyelids would be pulled so tight the eyes would barely close. Brows were lifted to high that women would walk around with a permanent look of surprise on their faces. Removing fat through surgery may get rid of bulges, but can actually make a person look older by giving a hollow appearance. Although, this type of surgery would change a person’s appearance, it did not make them look younger.

As people age, we lose facial fat and facial bony volume. This leads to a “deflation” of the face. We do not grow extra skin and we do not actually grow extra fatty deposits. Instead, as facial bone and fat shrinks, the skin begins to sag. The upper eyelids and brow begin to fall. The fat of the upper cheek falls and makes the bony area below the eye socket more prominent and hollow areas develop below the eyes. We delvelop lines between the nose and cheeks (Naso-Labial folds) and deep lines below or mouths (marionette lines).

Newer concepts in facial rejuvenation now address the idea that deflation is a major cause of an aged appearance. Upper eyelids and brows can be rejuvenated by restoring the fat pads in the eyebrows. Lower eyelids can also be rejuvenated by adding fillers. Adding volume to the cheek bones can restore fullness to the entire upper face and even reduce sagging of the jowels.

In the past there were few choices in fillers. Now there a literally dozens of choices when it comes to filling the face. Restylane and Juvederm are the most popular hyaluronic acid fillers. They are made of a natural gel that are best at filling superficial lines and the hollow area underneath the eyelids. They can also be used for making lips fuller. Thicker Hyaluronic Acids such as Perlane and Juvederm Ultra Plus are better at filling the deeper lines between the augmenting cheek bones. The injections can be administered in under an hour with the use of ice and a topical anesthetic to minimize pain. Bruising is typically minimal and can be covered with makeup. The results are immediate typically last 6-12 months.

Sculptra is another type of injectable that adds facial volume by stimulating new collagen growth. This type of injectable does not give instant results, but rather builds up volume with a series of injections over two to three months. It gives a natural overall youthful look that can last for up to two years.

In some people, injectables alone are sufficient to give a youthful look. Other people may still require surgery, lasers or other skin treatments to obtain the best possible results. Experienced surgeons are able to analyze the individual patients needs to offer the best combination of injectables, skin treatments and surgery. Keeping all of this in mind, treatments can actually restore a natural, youthful appearance with minimal risk and minimal downtime.

At my most recent in-office event, held on Thursday, November 11th, I performed a Live Demo on real patients of my practice for those who attended. They got to see Sculptra and Botox injections performed in just minutes with no downtime for the patient whatsoever.

Around this time of year, we can start to feel the effects of spending less time outdoors and the decrease in daylight hours. Our skin begins to really feel dry from the weather and the indoor heat. Are you feeling the Winter Blahs? You are not alone!

In addition to running a Special Event in my office in an effort to help you chase away those Winter Blahs and get you ready for Valentine’s Day, here’s a bit of advice on how to minimize the blues this winter.

Aside from the pick-me-ups you can get in my office what are some other ways to combat the blues we feel from the long Winter?

  • Keep up the exersize! Exersize is a great way to release those endorphins that make us feel good.
  • Try to get out in the sun for 15-20 minutes a day. Some sun exposure can be beneficial as it is a great source of Vitamin D!
  • Limit your alcohol intake. A glass of wine is fine but remember that too much alcohol disrupts your sleeping patterns and causes dehydration – not good for your skin!
  • Drink plenty of water. I’ve said it before and I’ll say it again – water moisturizes your skin from the inside-out. Try to drink 24 ounces a day.
  • Spend time with friends, especially those that make you laugh – a good laugh is the most fun way to do a crunch and work your abs. ;)

A very surgical Thanksgiving………

Having finished a very busy month in the office, I was looking forward to a few days of R & R in Florida. I didn’t know that it would be a working vacation. As my wife very meticulously prepared the 25lb turkey for the long stint in the oven, she noticed a 3 inch laceration in the skin on the breast. Afraid that it would lead to an unprecedented dry turkey, we quickly got out my mother-in-law’s sewing kit.

I urgently sewed up the laceration and while I was at it, my wife asked me to truss the bird and close up the cavity where she’d put the secret stuffing.

When company arrived, I was summoned by Cousin Stacey to the bathroom for a 20 minute consult. I advised her to have a lower-lid blepharoplasty, chemical peel and Juvaderm injections and got back to the business at hand….I am also the resident turkey-carver.

A side note – the turkey came out perfect. My Thanksgiving tip: The best, most juicy turkeys always flirt just a little with Salmonella. Remove the turkey as soon as the thermometer 140 degrees!

I am now happily back in New York and looking forward to the work week ahead.
