Is Blepharoplasty Covered by Insurance? What New York Patients Need to Know

One of the first financial questions patients ask when they’re considering eyelid surgery is a simple one: will insurance cover any of this? The answer is: it depends β€” but in some meaningful cases, it might. Understanding the distinction between cosmetic and functional blepharoplasty is the key to knowing whether coverage is a possibility for you.

Cosmetic vs. Functional Blepharoplasty

Cosmetic blepharoplasty is performed to improve the aesthetic appearance of the eyelids β€” reducing puffiness, correcting heaviness, opening up the eyes for a more refreshed look. These procedures are considered elective, and insurance plans do not cover them.

Functional blepharoplasty is performed to address a medical problem β€” specifically, when excess upper eyelid skin droops far enough to interfere with a patient’s field of vision. When the overhanging skin physically obstructs how much a patient can see, the surgery moves from the cosmetic category into a medically necessary one.

If you’ve been unconsciously raising your eyebrows throughout the day to lift your lids and see clearly, or if your upper eyelids have begun to touch or drape over your lashes, you may have a functional component to your eyelid concerns.

The Visual Field Test: How Coverage Is Determined

The standard pathway for insurance coverage of upper blepharoplasty involves a formal visual field test β€” also known as visual perimetry. This objective test measures the full extent of your peripheral vision, including how much of your superior (upper) visual field is obstructed by overhanging eyelid skin.
Most insurance carriers require the visual field test to be performed under two conditions: first with the eyelids in their natural, resting position, and then with the lids taped up to simulate the effect of surgery. If the taped results show a meaningful improvement in the visual field β€” typically a 12-degree or greater improvement, though thresholds vary by carrier β€” the surgery may qualify for coverage.
At Schlessinger Eye & Face, visual field testing can be arranged as part of the evaluation process. Dr. Schlessinger is experienced in working with insurance carriers and can help determine whether your situation meets the clinical criteria for a coverage claim.

What Documentation Is Required?

Insurance authorization for functional blepharoplasty typically requires:

  • A formal visual field test showing measurable obstruction at baseline and improvement with lid elevation
  • Clinical photographs documenting the degree of lid drooping
  • A letter of medical necessity from Dr. Schlessinger outlining the functional impairment
  • Confirmation that the procedure addresses vision, not cosmetic appearance

Our team is familiar with the requirements across major carriers and will work with you to compile a complete, well-supported authorization request.

What About Ptosis?

Ptosis β€” a condition in which the eyelid itself droops due to a weakness or detachment of the levator muscle rather than simply excess skin β€” is a separate diagnosis and is frequently covered by insurance when it meets clinical criteria. If Dr. Schlessinger identifies true ptosis during your evaluation, that will be discussed and documented separately. The distinction matters because ptosis repair involves different surgical techniques than standard blepharoplasty.

Financing for Cosmetic Blepharoplasty

For patients whose eyelid surgery is entirely cosmetic β€” or for those who want to add lower blepharoplasty or other elective procedures β€” Schlessinger Eye & Face offers CareCredit financing. CareCredit is a dedicated healthcare credit line that allows patients to manage the cost of elective procedures with flexible payment plans, including promotional interest-free periods for qualifying applicants. Our team can walk you through the application process during your consultation.
